We just ordered a RoadID bracelet for our daughter who has peanut, tree nut, legume, and wheat allergies. A basic ID is about $20, and the metal tag is interchangeable with their other products. They even have a shoeID which is pretty cool. They are designed for athletes (in case you have an emergency while you're on the road with no ID). It looks great and you can fit a lot of information on it, too. It's my favorite ID at the moment.
Her ID has this much information on it:
Her Full Name and Birth Year
My name and cell # MOM
DH's name and cell # DAD
Allergy to Peanuts,
Tree Nuts, Soy, Legumes
Use Epi-Pen, Call 911

I like the idea of a shoe ID because a kid could wear it on their tennis shoes all the time and wouldn't even think about it. If you get a brightly colored holder, it will be noticeable to the right people.
